For Patients & Their Families
Patients are usually referred to Hospital for Special Care from an acute care hospital. A physician, social worker or discharge planner makes the referral by calling HSC's Admissions Office at 860-827-4841 or 800-220-7723. The admission process is usually completed within 24 hours after receipt of a referral.

Hospital for Special Care Foundation
Formally established as an affiliate of the Center of Special Care in 1999, the Hospital for Special Care Foundation, Inc. is a nonprofit organization that raises charitable funds for the benefit of Center of Special Care and its nonprofit affiliates-Hospital for Special Care, HSC Community Services, Inc., and Brittany Farms Health Center.
