At HSC, an interdisciplinary team led by a board-certified internist provides a wide range of services designed to maximize the outcomes of patients who no longer need invasive or acute diagnostic procedures, but who continue to require 24-hour nursing care and/or monitoring.

Two tracks of care help accommodate the varying intensity of services required to treat these individuals. We also provide comprehensive care for individuals who require complex wound management.

• Complex Rehabilitation - The main focus of your program will be rehabilitation to achieve your functional goals, with management of your complex medical needs.
• Complex Medical- the main focus of your program will be management of your complex medical needs, also focusing on your functional goals to return home.
