Brittany Farms Health Center, part of the continuum of care offered by the Hospital for Special Care, is a Center of Excellence for Subacute Rehabilitation and offers services in long-term care, dementia and outpatient rehabilitation.

We provide a personalized approach to meeting the needs of patients and their families during the transition from home or an acute care setting into our subacute or long-term care services.

Our nurse evaluators are present and available in all of Connecticut's hospitals. Evaluators work closely with discharge planners and families. To determine that patients are referred to the best and most appropriate provider, they will answer questions about Brittany Farms Health Center and will help with the application process.
