Services for Medically Complex Patients
These are services designed to maximize the outcomes of patients who are no longer in need of invasive or acute diagnostic procedures yet continue to require 24-hour nursing care and/or monitoring. These patients may receive up to an hour of rehabilitation therapies per day. The two tracks in this program accommodate the varying intensity of services required to treat this population, with track II reflecting a higher acuity and higher level of reimbursement than track I.
Rehabilitation Track I
Designed for patients with a primary rehabilitation diagnosis, usually with a co-morbidity, who need less intense services than provided by acute rehabilitation.
When appropriate to a patient's rehabilitation plan, Hospital for Special Care provides additional services including audiology, continuous ambulatory peritoneal dialysis, dental, laboratory pharmacy, radiology and neuro-optometry. A daily living apartment offers patients and families the opportunity to simulate the home environment before discharge.
