General Rehabilitation
People come to HSC after they've had a serious injury or accident, because they've had a stroke, or they have complicated medical problems. While these may be life-changing occurrences, our team believes you can go home again. The hallmark of our approach is teamwork, and we are dedicated to rebuilding lives.
Doctors, nurses and therapists work together and in partnership with patients and their families. Together, they create individualized goals for every patient. As a patient's needs change, goals and plans are revised. It is hard work. HSC's Rehabilitation Services promotes patient independence and enhances every patient's abilities with a variety of inpatient programs and outpatient services, including a work injury rehabilitation program.
All patients and families are involved in comprehensive discharge planning that begins prior to admission and continues throughout the hospital stay. We follow up with patients and families approximately a week after discharge and again in three months. After discharge, patients, caregivers, family, and friends are encouraged to attend monthly education and discussion groups held at the hospital.
Patients may take advantage of our new Aquatic Rehabilitation Center, which includes fully accessible therapeutic and fitness pools as well as a fitness center for people of all abilities. HSC's Brittany Farms Health Center is a 284-bed skilled nursing facility recognized for excellence in the subacute rehabilitation.
